Schema Markup for Multiple Locations
A business with several locations gets one LocalBusiness block per location, each on that location's own page with its own address, phone, hours and URL. Paste your list of branches below and each one gets its block.

Copy the cells, header row included, and paste here. Columns: name, street, city, state, zip, phone, url, hours. Tabs or commas both work.
Paste it into the page's <head>, then check it in Google's Rich Results Test.
How the three sample branches are marked up
The sample list holds three dental offices. Each row becomes its own Dentist block: Downtown's block has 120 Main Street and its page https://www.example.com/downtown; Westside's has 45 Oak Avenue and two sets of hours, Monday to Thursday 7:00 a.m. to 4:00 p.m. and Friday 7:00 a.m. to noon. Each block's @id is its page address plus #business, so the three never merge into one.
Put Downtown's block on the Downtown page, and so on. A single "Locations" page that lists every branch may carry all of the blocks.
Linking branches to the company
To say the branches belong to one company, add parentOrganization to each block by hand, pointing at the company's own @id:

Questions
Can one page hold several LocalBusiness blocks?
Yes. A locations page may carry one block per branch. Each still needs its own name and address, and ideally its own url pointing at that branch's page.
Should every branch use the same name?
Use the name each branch shows on its Google listing. Many chains add the area, as in "Example Dental Westside", which also keeps the blocks easy to tell apart.
What columns does the list need?
A header row naming the columns, in any order: name, street, city, state, zip, phone, url and hours. Hours are written like "Mo-Fr 08:00-17:00; Sa 09:00-14:00".
